• Initial appointment with one of our Naturopathic Physicians. This will include health history and individualized meal/nutrition planning
• 4 follow-up appointments
• Weekly weigh in with each follow-up appointment
• Body measurements completed at week 1, 3, and week 5 (final week)
• 4 injections of Lipotropic factors and B12. These will occur at appointments 2, 3, 4, and 5
• COST:
  • Cash patients: 
    • $399 total for all 5 appointments including 4 injections
    • Supplements are an additional cost
  • Insurance billing patients:
    • Injection price: $20 per injection.
    • Copay/coinsurance for each appointment is required per insurance plan
    • Supplements are an additional cost

SILVER PLAN:

• Initial appointment with Naturopathic Physicians. This will include health history and individualized meal/nutrition planning
• 4 follow-up appointments
• Weekly weigh in with each follow-up appointment
• Body measurements completed at week 1, 3, and week 5 (final week)
• 4 injections of Lipotropic factors/B12/Biotin. These will occur at appointments 2, 3, 4 and 5
• This plan adds biotin injections to your 4 weekly injections. 
• COST:
  • Cash patients: 
    • $429 total for all 5 appointments including 4 injections
    • Supplements are an additional cost
  • Insurance billing patients:
    • Injection price: $30 per injection
    • Copay/coinsurance for each appointment per insurance plan
    • Supplements are an additional cost

GOLD PLAN (only offered in Portland):

• Initial appointment with one of our Naturopathic Physicians. This will include health history, individualized meal/nutrition planning, and acupuncture
• 4 follow-up appointments
• Weekly weigh in with each follow-up appointment
• Body measurements completed at week 1, 3, and week 5 (final week)
• 4 injections of Lipotropic factors/B12/Biotin. These will occur at appointments 2, 3, 4, and 5.
• This plan adds 4 acupuncture appointments. Each acupuncture visit will be 30 minutes long and individualized for each patient. 
• COST:
  • Cash patients: 
    • $549 total for all 5 appointments including 4 injections with 4 acupuncture appointments
    • Supplements are an additional cost
  • Insurance billing patients:
    • Injection price: $30 per injection.
    • Copay/coinsurance for each appointment per insurance
    • Supplements are an additional cost
